1. What is the metric?

This metric records the percentage of Short-Stay Residents Who Were Re-Hospitalized after a Nursing Home Admission.

  1. What does the metric mean?

    This metric measures the percentage of patients who entered or reentered the nursing home from an acute care hospital and were readmitted to a hospital for an inpatient stay or observation stay within 30 days of the start of the nursing home stay. The measure includes planned and unplanned hospitalizations.


  1. How do we calculate the metric?

The Numerator is the number of inpatient hospitalizations occurring within 30 days of a patient's entry or reentry into the facility from a hospital.

    An inpatient hospitalization is any of the following:

  1. A patient who was discharged to a hospital inpatient unit in PCC’s census ADT.

  2. A patient who was discharged to an Emergency Department in PCC’s census ADT and:

    1. Had an outcome of "Admitted, Status Uncertain," "Admitted, Observation," or "Admitted, Inpatient" entered in PCC.

            OR

  1. Had no outcome entered in PCC and did not return to the SNF within 72 hours of discharge to the ED

  1. Was discharged to the community and returned to the SNF from a hospital.


The Denominator is the number of all patients who entered or reentered the facility from an acute care hospital during the selected date range.


This metric is expressed as a percentage.
