Overview


The Hot List Report enables users to view the acuity of residents based on the residents’ triggering of specific criteria. 


Filters and Actions


Below is a view of the Hot List Report with highlighted areas to illustrate how to use the report.



Filters


The green box at the top left contains drop-down menus that allow users to filter by Division, Region, Facilities or Alert Type.


The blue box at the top middle allows users to filter by time period, with the option to choose between viewing the last 24, 48, or 72 hours.


The yellow box in the middle left is a search bar where users can search by residents’ name, MRN, or by facility.


Actions


The black box in the upper right corner has buttons that allows the user to export the data or print the data from the report.


The red box at the bottom is the report itself, and it can be ordered by any of the fields by clicking on any of them.


Criteria


The Hot List Report has 9 criteria used in determining the acuity of each resident. They are: eInteract, Clinical Alerts, Priority Progress Notes, Antibiotic Medications, Psychotropic Medications, Convergence Bypass, High Risk Convergence, Weight, and Vitals. Their sources and details are below.


  1. E-interact

    


Any resident with an e-interact alert on the Clinical Dashboard will trigger this category.

  1. Clinical Alerts



Any resident with a Clinical Alert on the Clinical Dashboard  will trigger this category.


  1. Priority Progress Notes



Any resident with a progress note under the High Priority Progress Notes tab will trigger this category.


  1. Antibiotic Medication



Any patient with a new order for an Antibiotic Medication within the past 24 / 48 / 72 Hours will trigger this category. These patients are shown on the Orders tab of the Clinical Dashboard (as above).


  1. Psychotropic Medication



Any patient with a new order for a Psychotropic Medication within the past 24 / 48 / 72 Hours will trigger this category. These patients are shown on the Orders tab of the Clinical Dashboard (as above).


  1. Convergence Bypass


Any resident who has been transferred to a Hospital in the last 24 / 48 / 72 hours will trigger this category. This is an externally calculated alert and provides the name of date, time, and name of the facility the resident was transferred to.


  1. High Risk Convergence



Any resident with a Convergence Progress Note in the Progress Notes tab within 24 / 48 / 72 hours will trigger this category.


  1. Weights/Vitals



Any resident with an alert for exceptional weights or vitals showing on the Weights and Vitals Portal will trigger this category. Vitals include: Blood Pressure, Temperature, Pulse, Respiration, Blood Sugar, O2 Sats, and Pain Level.
