Introduction

Resident Selection

Residents Included

Residents who are flagged on their vitals for either a 5% weight loss over 30 days and/or a 10% weight loss over 180 days and are not on a physician prescribed weight loss regimen based on a dietary assessment within the last 90 days.

Business Rules

Tasks:

  1. Resident’s Nutritional needs assessed by Dietician within the past 90 Days (Q1301)

    1. Location in PCC:

      1. Assessments → Dietary Nutritional Assessment

    2. Business Rule:

      1. IF a resident flagged on their vitals for either a 5% weight loss over 30 days and/or a 10% weight loss over 180 days

THEN the resident must have a recent Dietary Nutritional Assessment, completed within the last 90 days by a dietician

  1. Due Date:

    1. Within 7 days of the alert date

  2. Is classified as missed on the report:

    1. N/A

  3. Is no longer relevant to the report:

    1. If the resident has been discharged.

    2. 14 days after the alert date

  1. Resident’s Nutritional needs (Q1310 - calories, Q1311 - protein requirement, Q1312 - weight loss, Q1313 desired weight range) assessed by Dietician within the past 90 Days

    1. Location in PCC:

      1. Assessments → Dietary Nutritional Assessment

    2. Business Rule:

      1. IF a resident flagged on their vitals for either a 5% weight loss over 30 days and/or a 10% weight loss over 180 days

THEN the most recent Dietary Nutritional Assessment, completed within the last 90 days by a dietician must address the following:

  1. the prescribed caloric intake of the resident

  2. the prescribed protein intake of the resident

  3. the percentage of weight the resident has recently lost

  4. the desired weight for the resident

  1. Due Date:

    1. Within 7 days of the alert date

  2. Is classified as missed on the report:

    1. N/A

  3. Is no longer relevant to the report:

    1. If the resident has been discharged.

    2. 14 days after the alert date
